If you want to add or remove any members of your team roster, you must complete the form below IN FULL.  For adding players to your roster, you MUST provide their name and User ID email.  If they do not have a User ID email, they must create one here before being added to the roster.

You can only remove three players and add three players with each submission

Roster changes will be completed within two weeks of submission at the latest.
